Job summary

Claro Disability Services in Melbourne’s southeast is seeking a compassionate Support Worker to assist clients in accommodation settings. You’ll work with individuals who have complex needs, providing person-centered care, mobility support, daily tasks, and meaningful activities.

This role emphasizes collaboration with families and health teams, flexible shifts, and culturally respectful practice to help clients live more independent and connected lives.

Responsibilities

• Chat with the client and family to understand goals and routines.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

• Additional competencies required: Manual Handling/ceiling hoist experience, Challenging Behaviour, PEG feed experience
