Job summary

Just Buzz is seeking a compassionate and experienced Support Worker to join our team to provide practical, community-based support to people with disability and their families in Sydney.

You will work with clients, families, NDIs providers and allied health professionals to identify needs, develop tailored support plans and assist clients to access services that promote wellbeing, independence and participation.

Qualifications

• A minimum Diploma of Community Services or closely related qualification.
• At least 1 year of relevant work experience in family support or disability services.
• Ability to liaise with families, community organisations, government agencies and service providers.
• Strong case management, record-keeping and report-writing skills.

Responsibilities

• Meeting with individuals and families to identify their circumstances, goals and barriers to independence and wellbeing.
• Developing, implementing and regularly reviewing individualized support plans in consultation with clients and families.

The successful applicant will work closely with clients, families, community organisations and other service providers to identify individual needs, develop appropriate support strategies and assist clients in accessing services and resources that promote their wellbeing, independence and participation in the community.

Developing, implementing and regularly reviewing individualized support plans in consultation with clients, families, careers and relevant professionals. Providing practical and community-based support to people with disability to assist them in developing independence, confidence and participation in everyday activities.

Supporting clients with disability to access community activities, appointments, education, employment opportunities and other services in accordance with their individual support goals. Working closely with families and careers to provide guidance, information and practical strategies for supporting family members with disability or additional support needs. Monitoring clients' progress against their individual goals, identifying changes in support requirements and maintaining accurate case notes and progress reports.

Identifying social, emotional, behavioral and practical challenges affecting clients and working with them and their families to develop appropriate support strategies. Identifying when clients require specialised assistance and facilitating referrals to appropriate disability, health, housing, education, welfare and community service providers. Liaising with NDIS providers, support coordinators, allied health professionals, community organisations, government agencies and other service providers to coordinate appropriate client support.

Assisting clients and their families to understand and access available disability and community support services and other relevant community resources. Encouraging clients to develop daily living, communication, social and community participation skills that promote greater independence. Advocating for clients and families to ensure their needs, preferences and support requirements are appropriately communicated to relevant organisations and service providers.

Identifying potential risks to client wellbeing and escalating concerns in accordance with organisational safeguarding, incident management and reporting procedures. Maintaining confidential and accurate client records, support plans, progress reports, incident documentation and other records in accordance with organisational and regulatory requirements. Contributing to the review and improvement of family, disability and community support programs to ensure services continue to respond effectively to client needs.

Qualification & experience A minimum Diploma of Community Services, Individual Support, Disability Support, or a closely related qualification. A minimum of 1 year of relevant work experience in family support, community services, disability support, welfare services or a related field. Demonstrated experience assessing client needs and delivering individualised support.

Good understanding of community services, referral pathways and available support resources. Strong interpersonal and communication skills, with the ability to work effectively with clients from diverse backgrounds. Ability to liaise professionally with families, community organisations, government agencies and other service providers.

Strong case management, record-keeping and report-writing skills. Ability to work independently while contributing effectively within a multidisciplinary team. A compassionate, professional and client-focused approach to service delivery.
