Job summary

Healthii People in Campbelltown, Sydney NSW (Hybrid) is recruiting a Disability Support Worker to provide person-centred support to individuals living with disability, in homes and community settings. You will work under Care Group Community Services, delivering daily living assistance, personal care, and participation in community activities.

The role requires experience with complex behaviours, a calm, trauma-informed approach, and a commitment to dignity, respect and participant choice.

Responsibilities

• Provide high-quality, person-centred support to participants with complex behaviours.

• Implement Positive Behaviour Support Plans (PBSPs) and individual support plans.
• Respond confidently and professionally to challenging behaviours using trauma-informed approaches.
• Support participants to achieve personal goals and engage in meaningful community activities.
• Maintain accurate and timely case notes, incident reports and documentation.
• Work collaboratively with families, allied health professionals and the wider support team.
• Promote a safe, respectful and inclusive environment for participants and colleagues.
• Encourage development of life skills, emotional regulation and social inclusion.
• Follow organisational policies and safeguarding requirements at all times.
• Participate in team meetings, training and ongoing professional development.
• Maintain professional boundaries while providing compassionate, consistent support.

About the Role:

As a Support Worker with Care Group Community Services, you’ll provide person-centred support to individuals living with disability, empowering them to achieve their goals and maintain their independence. You’ll work closely with participants in their homes and the community, assisting with daily living, community participation, personal care, and implementing individual support plans.

This role involves supporting participants with complex behaviours, so you’ll use a calm, compassionate, and trauma-informed approach to build positive relationships, promote safety, and respond confidently to challenging situations. Working as part of a supportive team, you’ll play a vital role in helping participants live meaningful and fulfilling lives while upholding dignity, choice, and respect.

Your role will include:
