Job summary

Home Group is seeking a dedicated individual to support customers with complex needs to live more independently. You will lead planning, coordinate staff, and work with multi-agency teams to safeguard and empower clients, while helping them progress towards moving on into the community.

The role emphasizes person-centred care, collaboration, and resilience, with occasional work-from-home flexibility for administrative tasks. A Level 3 Diploma in Care is required or you are willing to pursue it.

Qualifications

  • Passion and experience in supporting others in a housing setting.
  • Level 3 Diploma in Care or equivalent (or willing to work towards it).
  • Experience assessing referrals and coordinating support.

Responsibilities

  • Lead support planning and coordinate our Support Workers to deliver person-centred support.
  • Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
  • Support Customers with daily tasks like budgeting, cooking and attending appointments.
  • Work with multi-agency teams to safeguard and empower our customers.
  • Help customers build independence and prepare for their move-on into the community.

What You’ll Do

You have
  • Passion and experience in supporting others ideally in a supported housing setting
  • Level 3 Diploma in Care or equivalent experience (or are willing to work towards it)
  • Experience assessing referrals and coordinating support
  • The ability to work under your own initiative, remain calm under pressure and have a resilient approach

The practical bits
  • You’ll work Monday to Friday, you’ll also have a flexibility to work from home occasionally to focus on administrative tasks when needed.
  • Optional sleep-in shifts may be available from time to time,
  • At North Tyneside LD, you will work with customers and coordinate staff to deliver the best quality of support possible.
  • Although you’ll be lone working, we’ll meet regularly to discuss support.
  • You need an Enhanced with barring list (we pay)
  • You’ll work across two services, Beacon and Stephenson Street, located within 5 minutes walk of each other, you’ll also have an opportunity to occasionally support customers at our outreach properties.
