Job summary

IUIH is seeking a Clinical Care Coordinator – MATSICHS Social Health to provide advanced clinical assessment, triage, and coordination of supports for Aboriginal and Torres Strait Islander clients with complex social and mental health needs.

The role requires high-level clinical judgement, collaboration with Social Health practitioners, and leadership in intake, triage, and care planning to ensure safe outcomes for clients within Caboolture, Morayfield, Strathpine area and beyond.

Qualifications

• Bachelor of Human Services/Social Science or Bachelor of Social Work and relevant registration.
• Min three years’ relevant experience.
• Current Criminal History Check.
• C Class Drivers Licence (QLD) and Working with Children’s Check or AHPRA Registration.

Responsibilities

• Act as central entry point for referrals to Social Health services.
• Conduct biopsychosocial assessments, including risk assessments.
• Apply advanced clinical judgement to triage and prioritise referrals based on acuity and complexity.
• Deliver evidence-based brief interventions (e.g. CBT-informed, trauma-informed approaches).
• Manage a caseload of clients with complex needs, including structured follow-up and review.
• May be required to work across Caboolture, Morayfield, Margate and/or Deception Bay from time to time.

About the role

As a Clinical Care Coordinator – MATSICHS Social Health, your role is to provide advanced clinical assessment, triage, and coordination of supports to Aboriginal and Torres Strait Islander clients experiencing complex social, emotional wellbeing and mental health challenges.

You will exercise high-level clinical judgement to assess risk, prioritise service responses, and deliver brief evidence-based interventions. The role works collaboratively with Social Health practitioners and provides clinical leadership in intake, triage, and care planning to ensure safe and effective client outcomes.

Your day to day duties may include:

• Act as a central entry point for referrals to Social Health services.
• Conduct comprehensive biopsychosocial assessments, including risk assessments (suicide, self-harm, harm to others).
• Apply advanced clinical judgement to triage and prioritise referrals based on acuity and complexity.
• Deliver evidence-based brief interventions (e.g. CBT-informed, trauma-informed approaches).
• Manage a caseload of clients with complex needs, including structured follow-up and review.
• May be required to work across Caboolture, Morayfield, Margate and/or Deception Bay from time to time.

About you

• Current Criminal History Check.
• Current C Class Drivers Licence (QLD).
• Current Working with Children’s Check (Qld Blue Card) or AHPRA Registration.
• Bachelor of Human Services/Social Science or Bachelor of Social Work
• Minimum three years’ relevant experience.
