As Care Coordinator CCSS – MATSICHS, your role is responsible for specialist community and primary health nursing services that focuses on the prevention, early detection and management of health problems for Aboriginal and Torres Strait Islander people.
The role will work collaboratively with patients and designated general practices to facilitate a person with a chronic condition to manage their health in a way that will result in an optimal health outcome. As a member of a multi-disciplinary team ensure effective screening, service delivery and administration practices are delivered in accordance with the IUIH Model of Care.
The role will entail service delivery from designated primary clinic locations and outreach services, under limited supervision, as required.
