Job summary

IUIH is seeking a Care Coordinator for the Staying Deadly Hub in Caboolture. The role provides outcome-focused health, social and practical support to Aboriginal and Torres Strait Islander clients within a multidisciplinary team.

You will manage a caseload of clients with mental health needs, work in a trauma-informed recovery approach, and support transitions from inpatient or other services, including home visits to sustain wellbeing.

Qualifications

• Aboriginal and/or Torres Strait Islander person (essential)
• Current Criminal History Check
• C Class Driver's Licence (QLD)
• Current Working with Children’s Check (Qld Blue Card)
• Current CPR and First Aid Certificate
• Certificate IV or higher in Aboriginal and/or Torres Strait Primary Health or willingness to complete
• Experience in mental health, drug and alcohol, or related social services
• Qualification in Social Work/Mental Health/Behavioural Science or related field

Responsibilities

• Develop and maintain client support plans with clients and service providers
• Work with clients using trauma-informed, recovery/strengths-based approaches
• Coordinate services for clients with mental health, substance use, chronic disease and social health needs
• Conduct home, community and custody visits to support client recovery

Working within a multidisciplinary team to support clients with moderate to severe mental health needs that; are at risk of entering the acute care system if their symptoms are unmanaged, or who are stepping down from an inpatient episode of care to ensure they are well supported in the community through holistic and wrap around primary health and mental health care. The role will manage a caseload of clients with varying needs, to empower and foster strength, resilience and well-being through participation in intensive case management, planning and coordination. Your day to day duties may include:* Developing and maintaining support plans in conjunction with clients and relevant service providers.* Work with clients in a trauma informed, recovery and/or strengths-based approach to formulate, implement and review well-being plans including a comprehensive assessment of need and safety planning.* Assisting in the coordination and delivery of services for clients identified with drug and alcohol, mental health, chronic disease and social health issues.* Conduct home, community and/ or custody visits to support ongoing client recovery.* Contributing to smooth transitions for client who are accessing other services. **About you****To perform this role, it is essential that the incumbent is an Aboriginal or Torres Strait Islander person.
