Job summary

Ord Valley Aboriginal Health Service (OVAHS) in Kununurra, WA, is seeking a Specialist Support Coordinator to design and monitor complex service plans across health, disability, housing and justice sectors. • Bachelor's degree in a relevant field.
• 2–3 years' experience managing complex cases in high-intensity community, disability, or health services.
• Commitment to culturally safe and inclusive practices.

Responsibilities

• Design, implement, and monitor complex service plans across health, justice, housing and disability sectors.
• Build participants' capacity to self-manage supports and promote independence.
• Foster strong networks to connect participants with quality supports reflecting individual needs and goals.
• Focus on reducing barriers to care and building long-term capacity for self-direction.
