Job summary

Latrobe Community Health Service in Morwell, VIC, is seeking a compassionate Social Support Group Facilitator & Meal Coordinator on a permanent part-time basis (60 hours per fortnight).

You will coordinate social support group activities, oversee nutritious meals, manage volunteers and transport, and ensure records and compliance are met while delivering high‑quality outcomes for older adults and the broader community.

Qualifications

• Certificate IV or Diploma in related care field.
• Current First Aid Certificate (Level 2).
• Current CPR Certificate.
• Food Safety Supervisor Certificate (or willingness to obtain).
• Current Victorian Driver Licence is desirable.

Responsibilities

• Coordinate, facilitate, and deliver Group Social Support programs.
• Support participant intake, goal planning, and wellbeing monitoring.
• Coordinate volunteers, transport services, and program resources.
• Oversee nutritious meal services and kitchen compliance.
• Manage administration, records, reporting, budgets, and documentation.
• Build positive relationships with participants, carers, staff, volunteers and partners.
• Contribute to quality improvement, compliance and service development.
• Adhere to organisational values and policies.

Make a real difference in the lives of older adults and vulnerable community members. We are seeking a compassionate, organised, and community-minded Social Support Group Facilitator & Meal Coordinator to join our team.

In this diverse role, you will coordinate social support group activities and oversee nutritious meal programs that promote participant wellbeing and community connection. Reporting to the Team Leader, Social Support Program, you will support participants, coordinate volunteers, transport, and program resources, maintain records and compliance requirements, and work collaboratively with staff, volunteers, and community partners to deliver high-quality services and positive participant outcomes.

Key Objectives

• Coordinate, facilitate, and deliver Group Social Support programs that promote independence, wellbeing, and social connection through a Wellness and Reablement approach.
• Support participant intake, goal planning, and wellbeing monitoring and referrals.
• Coordinate volunteers, transport services, and program resources to support effective service delivery.
• Oversee nutritious meal services, kitchen compliance with Class 2 Kitchen regulations, and food safety requirements.
• Manage administration, records, reporting, budgets, and service documentation.
• Build positive relationships with participants, carers, staff, volunteers, and community partners.
• Contribute to quality improvement, compliance, and continuous service development.
• Adhere to organisational values, policies, procedures, and legislative requirements.

What We Are Looking For

• Strong communication, organisational, and time management skills.
• Experience coordinating group activities and participant-centered programs.
• Ability to support older people and diverse community members with empathy and respect.
• Experience managing multiple priorities and operational tasks in a busy environment.
• Ability to work collaboratively with volunteers, staff, and stakeholders.
• Sound computer skills, including Microsoft Office and client management systems.
• Knowledge of Wellness and Reablement, aged care standards, and food safety practices.
• Commitment to high-quality services and continuous improvement.

Essential Requirements

• Current Victorian Driver Licence.
• Current Working with Children Check.
• Current First Aid Certificate (Level 2).
• Current CPR Certificate.
• Ability to drive a 12-seater Social Support bus.
• Certificate IV or Diploma in Individual Support, Ageing Support, Leisure and Health, Community Services, or a related field (or willingness to obtain).
• Food Safety Supervisor Certificate (or willingness to obtain).
