Job summary

Healthscope Independence Services is seeking a passionate Disability Support Worker to provide high-quality care in our SIL services in Geelong. You will assist with daily living, personal care, and community engagement, aligned with individual support plans.

The role requires a Certificate IV in Disability or related qualification and a minimum of 1 year in disability/healthcare. You will work in a supportive, mission-driven team with opportunities for training and development.

Qualifications

• Certificate IV in Disability, Community Services, or related qualification.
• Minimum of 1 year of experience in disability or healthcare services is required.
• Strong communication, written, interpersonal, and organisational skills.
• Flexibility to work across a range of shift types.

Responsibilities

• Provide personal care support in line with individual support plans.
• Ability to complete overnight sleepover shifts, remain on-site during the designated sleepover period, respond to participant needs if required, and assist with morning routines and handover.
• Assist participants with daily living activities in meal preparation and feeding.
• Assist with household tasks such as cleaning, laundry, etc
• Accompanying and supporting clients in community-based activities, such as social outings, appointments, and recreational pursuits
• Ensure all required documentation (including daily notes, communication and progress reports) is thorough and up to date.
• Experience with behaviours of concern
• Adhere to all organisational policies, including medication administration, WHS, client rights, privacy, and confidentiality.
• Work within a team environment.
