Job summary

Greater Western Aboriginal Health Service (GWAHS) seeks a Social & Emotional Wellbeing (SEWB) Link Worker to support Aboriginal clients in Mount Druitt and Western Sydney on a 6-month fixed-term basis. The role focuses on navigating mental health, AOD, and broader SEWB needs with culturally safe care.

The successful candidate will develop care plans, coordinate services, and maintain records, working closely with GWAHS teams and community partners.

Qualifications

• Tertiary qualification in Community Services, Mental Health, AOD or related field.
• Current NSW Driver Licence.
• Current Working with Children Check.
• National Police Check.
• Confirmation of Aboriginality.

Responsibilities

• Conduct initial and ongoing client assessments and planning.
• Develop individualised care plans in partnership with clients and families.
• Provide direct support and care coordination within SEWB model.
• Connect clients with mental health, AOD and community services.
• Maintain accurate records and contribute to case reviews.
• Collaborate with GWAHS clinicians and community programs.
• Support transport and practical assistance for clients.
