Job summary

Central Australian Aboriginal Congress Aboriginal Corporation seeks an SEWB Case Manager to provide trauma-informed, strengths-based case management for clients with complex needs. You will coordinate advocacy, outreach and life skills training, and connect clients to Congress services, while coordinating interagency teams.

The role is Aboriginal Identified, requires a NT Driver's Licence or ability to obtain one, and involves travel to remote locations as part of community-led health work in

Qualifications

• Aboriginal Identified position; must be Aboriginal and accepted by the community.
• Experience providing trauma-informed, strengths-based case management.
• Knowledge of Social and Emotional Wellbeing (SEWB) principles and practice.
• Ability to collaborate with internal and external providers.
• Maintain confidentiality in a health setting and demonstrate cultural sensitivity.

Responsibilities

• Provide case management to SEWB clients referred for support.
• Coordinate advocacy, outreach, skill development and life skills training.
• Facilitate access to Congress services and coordinate interagency meetings.
