Job summary

Ross Care in Prospect SA is seeking a dedicated Service Coordinator to coordinate and deliver high-quality disability support services. Responsibilities

  • Coordinate client appointments, home visits and support services per care plans.
  • Perform initial client assessments and develop tailored support plans.
  • Maintain accurate client records in line with policies and legislation.
  • Communicate with clients, families, carers and staff to ensure service clarity.
  • Monitor progress and adjust service delivery for optimal outcomes.
  • Liaise with external providers to coordinate integrated care.
  • Handle inquiries with empathy; escalate issues as needed.
  • Ensure compliance with disability support regulations and safeguarding.
  • Assist in training support workers when required.
  • Prepare management reports and QA data.

About the role

Ross Care is seeking a dedicated and compassionate Service Coordinator to join our team in Prospect SA on a full-time basis. In this pivotal role, you will be instrumental in coordinating and delivering high-quality disability support services to our clients. As a Service Coordinator, you will act as a key liaison between clients, staff, families, and external service providers, ensuring that individualised care plans are effectively implemented and that every client receives the support they need to maintain independence, dignity, and quality of life. This position is essential to our operations, directly impacting the wellbeing and satisfaction of vulnerable members of our community.

Key responsibilities

Coordinate client appointments, home visits, and support services in accordance with individualised care plans

Conduct initial client assessments to identify support needs and develop appropriate, tailored support plans

Maintain accurate and up-to-date client records and documentation in compliance with relevant legislation and organisational policies

Communicate effectively with clients, families, carers, and support staff to ensure clear understanding of services and expectations

Monitor client progress and adjust service delivery as required to ensure optimal outcomes and client satisfaction

Liaise with external service providers, healthcare professionals, and community agencies to coordinate integrated care

Handle client inquiries and concerns with empathy and professionalism, resolving issues promptly or escalating appropriately

Maintain compliance with disability support regulations, standards, and safeguarding requirements

Assist in the training of support workers as needed

Prepare reports and data for management review and quality assurance processes

What we’re looking for

Relevant qualifications or certification in aged care, disability support, community services, or a related field (or demonstrated equivalent experience)

Proven experience in a coordinating or supervisory role within aged care, disability support, or community services sectors

Strong organisational and time management skills with the ability to manage multiple clients and priorities simultaneously

Excellent written and verbal communication skills to liaise effectively with diverse stakeholders

Demonstrated understanding of disability support principles, including person-centred care and dignity of risk

Sound knowledge of relevant legislation and standards, including the Disability Discrimination Act, and privacy legislation

Proficiency with computer systems and database management software

A compassionate and empathetic approach to working with vulnerable populations

Current Working with Children Check, NDIS Worker Screening Check (or willingness to obtain)

Excellent problem-solving abilities and sound judgment in complex situations
