Job summary

Western Sydney Local Health District's Community Health team is seeking a Senior Social Worker to provide advanced clinical services to clients and families in Mount Druitt and Blacktown. You will manage complex psychosocial needs with autonomy and lead in clinical leadership and education.

You will also mentor staff and students, contribute to service development and quality improvement, and collaborate with multidisciplinary teams to improve outcomes for clients facing chronic illness,

Qualifications

• Degree in Social Work with eligibility for membership of the Australian Association of Social Work
• Ability to work with minimal supervision within a multidisciplinary team
• Experience or capability in developing policies and procedures for service improvement and staff training
• Strong time management and written/verbal communication skills
• Experience in quality assurance and clinical practice improvement activities
• Ability to work with consumers, families and carers under high family stress
• Good computer skills including Cerner eMR
• Australian driver’s license and willingness to use for work

Responsibilities

• Deliver expert social work assessment, intervention and care planning for clients with chronic, complex and palliative care needs
• Mentor and support social work staff and students to build workforce capability
• Contribute to service development, quality improvement and clinical excellence
• Collaborate with multidisciplinary teams, health professionals and external agencies to achieve optimal client outcomes
• Lead and participate in quality improvement initiatives and service planning across multiple community health locations
