NSW Health's Western Sydney Local Health District is seeking a Senior Social Worker to deliver advanced clinical social work services in community health settings, with a focus on chronic, complex and palliative care.

You will mentor staff and students, collaborate with multidisciplinary teams, and lead assessments, care planning and service improvement initiatives while supporting clients, families and carers through grief and end-of-life care.

Competenze

• Degree in Social Work with eligibility for membership of the Australian Association of Social Work.
• Ability to work with minimal supervision within a multidisciplinary team.
• Experience or capability in developing policies for service improvement and staff training.
• Strong time management and written/verbal communication, including report writing.
• Experience in quality assurance or clinical practice improvement.
• Ability to work with consumers, families and carers during high-stress periods.
• Valid Australian driver's license and willingness to use for work.

• Deliver expert social work assessment, intervention and care planning for clients with chronic, complex and palliative care needs.
• Manage complex psychosocial issues including grief, trauma, loss, child protection, domestic violence, elder abuse and guardianship matters.
• Provide clinical leadership, mentoring and professional support to social work staff and students.
• Lead and contribute to quality improvement initiatives, service planning, education and evidence-based practice activities.
