Job summary

Flinders Medical Centre’s Virtual Rehabilitation Ward, part of the Southern Adelaide Local Health Network, seeks a Senior Social Worker for a part-time contract until June 2027. You will provide specialist social work assessment, intervention and care planning for clients with complex needs to help them remain at home when possible, working within a multidisciplinary team.

The role requires an eligible Social Work degree, hospital-based experience, and commitment to high-quality, safe care

Qualifications

• A degree or equivalent in Social Work with eligibility for full membership of the Australian Association of Social Workers.
• Competence in applying a wide range of assessment and therapeutic interventions for clients with complex needs.
• Experience delivering social work services in a hospital setting.
• Understanding of National Safety and Quality Health Care Service Standards.

Responsibilities

• Independently assessing clients' abilities, problems and needs using formal and informal assessments.
• Communicating with clients' family, carers and other service providers.
• Participating in interdisciplinary clinical and planning activities to enhance the service.
• Initiating and contributing to educational activities within the team.
