Job summary

SA Health invites applications for a Senior Social Worker in the Virtual Rehabilitation Ward at Flinders Medical Centre. The role focuses on specialist social work assessment, intervention and care planning for people with complex medical and psychosocial needs to help them remain at home when appropriate.

Reporting to the Manager, you will work within SALHN frameworks, collaborate with families and the care team, and contribute to education and planning activities while delivering high-quality,

Qualifications

• Qualification in Social Work with eligibility for full membership of the Australian Association of Social Workers.
• Experience applying a range of assessment and therapeutic interventions for complex needs.
• Ability to analyse clinical care issues and contribute to quality activities or research.

Responsibilities

• Independently assess clients' abilities, problems and needs using formal and informal assessments.
• Communicate with clients' family, carers and other service providers.
• Participate in interdisciplinary clinical and planning activities; initiate educational activities within the team.

The Virtual Rehabilitation Ward at Flinders Medical Centre provides care to people with complex medical, functional, and psychosocial needs in their own home - in place of an extended or avoidable hospital admission.

About the role:

Reporting to the Manager, this role is responsible for providing specialist social work assessment, intervention and care planning to people with complex medical, functional and psychosocial needs - supporting them to remain in their own home in lieu of an extended or avoidable hospital admission.

• Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan. Independently assessing clients' abilities, problems and needs using a variety of formal and informal assessments as needed.
• Communicating with clients' family, carers and other service providers.
• Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to clients. Initiating, developing and contributing to educational activities within the team.

About you:

• A degree or equivalent in Social Work, which gives eligibility for full membership of the Australian Association of Social Workers Demonstrated competence in the application of a wide range of assessment and therapeutic interventions to clients with complex needs. Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
• Experience providing social work services to people in a hospital setting.
• Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards.
