Job summary

Concentric is redefining allied health in aged care, offering proactive, outcomes‑focused therapy at an onsite wellness centre. You will work closely with residential teams and community clients, delivering tailored wellness plans, rehabilitation programs and group sessions to improve strength, mobility, balance, cognition and overall quality of life.

This Senior Physiotherapist role in Plenty, VIC offers a hands‑on, innovative environment with mentoring, social events, and ongoing upskilling

Qualifications

• A Degree in Physiotherapy (or equivalent) is required.
• Current AHPRA registration is mandatory.
• NDIS Worker Screening Check required.
• Working with Children Check required.

Responsibilities

• Discover strengths and support each resident to thrive.
• Assess residents to identify mobility, safety and daily activity goals.
• Develop personalised therapy to enhance independence and function.
• Collaborate with residents, families and colleagues for person‑centred care.
• Document accurately and clearly, ensuring quality records.

You’ll work in a model that goes beyond standard aged care treatment, delivering tailored wellness plans, structured rehabilitation programs, and engaging group exercise and fitness sessions designed to improve strength, mobility, balance, cognition, and overall quality of life.

Our clinicians partner closely with residential teams while also supporting community clients, giving you the chance to run groups, provide targeted 1:1 therapy, and see real functional progress in your clients.
