Job summary

Concentric is seeking a dedicated Physiotherapist for Homecare in Mornington, Victoria. You will provide in-home assessments and personalized plans to improve function and independence, collaborating with clients and families for optimal outcomes.

The role offers a full-time schedule, mentoring, education programs, and a supportive team culture that values compassion, initiative, and professional growth.

Qualifications

• A Degree in Physiotherapy (or equivalent) and AHPRA registration.
• Willing to travel to Mornington and surrounding areas.
• NDIS Worker Screening Check required.
• Working with Children Check required.
• Car and valid Australian driver's licence.

Responsibilities

• Discover strengths and tailor homecare plans for clients in their homes.
• Assess clients in their homes to identify functional abilities and safety considerations.
• Design plans to boost independence and life quality.
• Collaborate with clients, families, and colleagues on person-centred care.
• Document clearly and maintain accurate records.
• Stay updated with latest research and best practices.
• Participate in team meetings and growth opportunities while engaging with the team.
