Job summary

Concentric is seeking a Physiotherapist to support aged care clients across Grovedale, Ocean Grove and Leopold in Victoria. The role focuses on comprehensive assessments, goal-driven plans, and outcomes-focused therapy within a supportive multidisciplinary team.

Qualifications

• Degree in Physiotherapy (or equivalent).
• Current AHPRA registration.
• Valid Australian driver’s licence.
• Willing to obtain NDIS Worker Screening Check.
• Willing to obtain Working with Children Check.

Responsibilities

• Conduct comprehensive assessments to identify functional abilities, limitations, and goals.
• Develop individualised treatment plans to maximise independence and quality of life.
• Provide evidence-based therapeutic interventions.
• Collaborate with clients, families, and a multidisciplinary team to create person-centred care plans.
• Communicate effectively with stakeholders.
• Deliver individual and group therapy to enhance daily living, mobility, and self-care.
• Provide education and support to clients and caregivers on physiotherapy interventions.
• Maintain accurate client records and documentation.
• Stay informed on latest research, practices, and regulatory changes.
• Participate in team meetings and professional development.

About your new role:

• Conduct comprehensive assessments of individuals to identify their functional abilities, limitations, and goals.
• Develop individualised treatment plans and intervention strategies to maximise independence, functional capacity, and quality of life.
• Provide evidence-based therapeutic interventions.
• Collaborate with clients, their families, and a multidisciplinary team to develop person-centred care plans and ensure holistic support.
• Communicate effectively with Stakeholders
• Deliver individual and group therapy sessions to enhance daily living skills, cognitive abilities, mobility, and self-care.
• Provide education and support to clients, caregivers, and support workers to enhance their understanding of physiotherapy interventions.
• Maintain accurate and up-to-date client records and documentation.
• Stay informed about the latest research, best practices, and regulatory changes.
• Participate in team meetings, case conferences, and professional development activities and contribute to a positive workplace culture.
