Job summary

Palliative Care South East Ltd. invites a Nurse Practitioner to deliver autonomous, NP-led palliative care in clients’ homes across the southeast Melbourne region. You’ll work with the Medical Officers, trainees, and the interdisciplinary team to deliver comprehensive assessment, diagnosis, treatment and care planning.

In this senior role, you’ll build capacity in residential aged care and support GPs to embed a palliative approach, including participation in a rotating on-call weekend roster.

Qualifications

Nurse practitioner or en route to NP with recent palliative care experience.
Proven advanced clinical skills in assessment, treatment and management of palliative/end-of-life needs.
Ability to work autonomously within a multidisciplinary team.

Responsibilities

Lead clinical care in clients' homes with NP expertise.
Provide consultative guidance to residential aged care facilities and GPs.
Support capacity-building for residential aged care staff and clinical teams.
