Job summary

Queensland Health's Gladstone Community Specialist Palliative Care is seeking aClinical Nurse to provide specialist, person-centred care to patients and families navigating life-limiting illness. You will work within a multidisciplinary team to support home-based care and coordinate services.

The role is permanent part-time (up to 32 hours per fortnight) with opportunities for professional development. Applicants should have current AHPRA registration and be comfortable delivering compassionate

Qualifications

Current registration as a Nurse with AHPRA.
Experience in palliative care or end-of-life care is desirable.
Ability to work autonomously within a multidisciplinary team.
Strong communication skills with patients and families.

Responsibilities

Perform comprehensive nursing assessments and develop care plans aligned to patient goals.
Manage Continuous Subcutaneous Infusions (CSCI).
Educate families to safely administer injectable medications at home.
Coordinate care, referrals and access to services across teams.
Utilise PCOC clinical assessments to recognise changes and escalate as needed.
Provide education and support to the broader palliative care workforce.
