Job summary

Eldercare is seeking a skilled Palliative Care Nurse Practitioner/Specialist to lead advanced palliative care practices across our residential aged care homes. This role is based at Head Office with travel across metro Adelaide.

You will provide expert clinical leadership, guidance and education to teams, collaborate with GPs and specialists, and champion evidence-based care to improve resident outcomes and experiences.

Qualifications

• Extensive recent experience in palliative care for adults/older persons.
• Current AHPRA registration as a Nurse Practitioner or Registered Nurse.
• NP endorsement for Nurse Practitioner and ability to practice autonomously.
• Proven clinical leadership, consultation and staff development.

Responsibilities

• Provide specialist clinical assessment, consultation and support for residents with palliative and end-of-life care needs.
• Support the implementation of Eldercare's palliative approach and evidence-based clinical practice.
• Build confidence, knowledge and capability within clinical and care teams through coaching, mentoring and education.
• Facilitate palliative care needs rounds and multidisciplinary discussions.
• Partner with GPs, specialist providers and multidisciplinary teams to support quality resident outcomes.
• Promote advance care planning and resident-centred decision making.
• Provide support and guidance to teams caring for residents with complex symptoms and changing care needs.
• Contribute to quality improvement initiatives, projects and service development opportunities across the organisation.
