Job summary

South Western Sydney Local Health District is seeking a motivated occupational therapist to join the Liverpool Transitional Aged Care Program (TACP) as a Level 3 clinician in a full-time permanent position.

You will conduct home assessments, set goals, and deliver client-centred treatment to older adults transitioning from hospital to community, collaborating with a multidisciplinary team to improve outcomes and quality of life.

Qualifications

• Recognised Degree in Occupational Therapy with AHPRA registration.
• Advanced clinical knowledge and substantial experience in Occupational Therapy.
• Experience planning, implementing and evaluating local service delivery with quality improvement.
• Strong organisational and time management skills, capable of supervising staff and students.

Responsibilities

• Conduct home assessments for clients.
• Contribute to goal setting and client-centred treatment.
• Support discharge planning and community reintegration.
• Participate in quality improvement and service development activities.
