Liverpool University Hospitals NHS Foundation Trust is seeking an experienced Complex Discharge Nurse to join the Complex Discharge Team. The role focuses on coordinating safe, timely and person-centred discharges for patients with complex health and social care needs, working with ward teams, community partners and local authorities.

The post holder will lead discharge planning across pathways, ensure EDDs are documented, and identify barriers to reduce delays while improving patient outcomes

Qualifikationen

• Registered Nurse to join the Complex Discharge Team.
• Experience in discharge planning, patient flow and case management.
• Ability to coordinate complex discharge pathways across health and social care systems.
• Strong communication and leadership skills.

Aufgaben

• Coordinate and manage the discharge planning process from admission to discharge for patients with complex needs.
• Ensure documented discharge plans, Expected Date of Discharge (EDD) and actions are in place.
• Identify and manage barriers to discharge to maintain patient flow.
• Lead multidisciplinary discussions and liaise with community services and Local Authorities.
• Support Home First and Discharge to Assess principles across pathways.
