Job summary

Department of Health, Tasmania is seeking an experienced allied health professional to join the Complex Care team at Launceston General Hospital. You will work with nursing, medical and allied health staff, and external providers to support patient journeys and improve outcomes for those with highly complex needs.

The role focuses on expert care management, complex discharge planning and hospital avoidance, with leadership beyond individual cases and opportunities to drive system-wide

Qualifications

Satisfactory completion of an approved allied health professional degree and registration with the relevant National Board.
Clinical experience in complex care or high-risk discharge planning.
Ability to work across multi-disciplinary teams and partners.
Postgraduate qualifications relevant to the position.

Responsibilities

Provide expert clinical advice on care management and system pathways.
Lead complex discharge planning and hospital avoidance strategies.
Collaborate with external service providers to support right place right care pathways.
Coach multidisciplinary teams and drive service improvement initiatives.
