Job summary

Court Services Victoria in Heidelberg is seeking an Advanced Case Manager to support people with complex needs within ARC and CISP. You will work in a multidisciplinary team to assess, plan and coordinate services, while mentoring staff to raise practice standards.

You will engage housing, mental health, AOD and family violence services, produce court reports, and contribute to program-wide improvements that enhance client outcomes.

Qualifications

• Diploma level qualification in a relevant field; degree level desirable.
• Experience with complex needs caseloads in areas such as mental health, disability, homelessness or substance use.
• Experience in coaching/mentoring case managers and improving practice.

Responsibilities

• Deliver advanced, strengths-based case management to clients with complex psychosocial needs.
• Conduct assessments and develop person-centred support and treatment plans.
• Coordinate access to housing, mental health, alcohol and other drug services, family violence interventions and related supports.
• Identify and manage client risks through risk management and harm minimisation.
• Prepare high-quality court reports and provide advice to the Court.
• Coach and mentor case managers with specialist knowledge.
• Build relationships with judiciary, legal professionals, community services and internal stakeholders.
• Contribute to continuous improvement initiatives to strengthen service delivery.
