Job summary

Care Connect is seeking a Senior Care Manager to lead a team of Care Managers within our not-for-profit home care organization. You will provide day-to-day leadership, coaching and practice guidance to ensure safe, high-quality, person-centred care that supports independence and wellbeing.

You will translate aged care reform, organisational priorities and quality standards into consistent practice, strengthening governance and service excellence across the team.

Qualifications

• Experience in care management, case coordination, team leadership or service management.
• Strong understanding of person-centred care, wellness and reablement principles.
• Experience coaching, mentoring and developing employees.
• Knowledge of aged care quality standards, safeguarding, care governance and risk management.
• Experience within aged care, home care, health, disability or community services.
• Leadership, coaching or case management qualifications.
• Excellent communication, stakeholder engagement and problem-solving skills.
• Strong organisational skills and the ability to manage competing priorities.

Responsibilities

• Lead, coach and develop a team of approximately 8-12 Care Managers.
• Provide guidance and support on complex client situations and escalations.
• Promote person-centred care, wellness and reablement principles.
• Strengthen quality, safeguarding, care governance and risk management practices.
• Support service performance, client experience and continuous improvement initiatives.
• Build team capability through coaching, mentoring and development.
• Partner with internal teams, providers, clients and families to achieve positive outcomes.
• Support the implementation of aged care reforms and new ways of working.
• Use data, feedback and insights to drive service improvements and operational effectiveness.
