You will work closely with clinical and non-clinical teams, supporting day-to-day operations of the Care Coordination function and signposting to appropriate services both within and outside the practice.

Responsibilities

• Provide a professional, respectful and efficient first point of contact for patients, visitors and healthcare professionals.
• Coordinate patient care and follow-up with GPs, nurses, clinicians and the multidisciplinary team.
• Undertake administrative and medical secretary duties including referrals, records and follow-up actions.
• Process prescriptions and respond to prescription queries in collaboration with clinicians.
