Job summary

National Disability Insurance Agency – Mount Isa QLD is seeking an APS5 Remote Planner to undertake planning activities and engage with participants, supporting plans and community supports in rural and remote communities.

The role involves offsite visits to participants’ homes, travel over long distances and working with internal and external stakeholders to resolve moderately complex enquiries and deliver outcomes that align with the Participant Service Guarantee.

Qualifications

Responsibilities

• Working with service providers and participants to gather and verify relevant information to facilitate access, plan development, ensure successful plan implementation and maintain community supports.
• Undertaking reviews of plans with participants, delivering and approving plans to meet targets.
• Developing outcome-based plans reflecting participant choice and control within a reasonable and necessary framework.
• Building participant capacity to maintain effective networks by liaising with individuals, families/ carers and their communities to improve natural and community supports.
• Providing information to members of the community about the NDIS, mainstream services and community supports to improve participation and inclusion.
