Job summary

St John of God Midland Private Hospital is seeking a Discharge Coordinator on a fixed-term full-time basis from 30 November 2026 to 1 August 2027. The role focuses on delivering patient-centred discharge planning and improving outcomes in collaboration with nursing leadership.

You will maintain NMBA competency, lead education for the multidisciplinary team, and advance clinical practice while supporting our Mission and Values.

Qualifications

• Registered Nurse/Midwife with NMBA registration.
• Demonstrated knowledge and leadership in the specialty.
• Experience with quality improvement initiatives and patient safety.

Responsibilities

• Deliver patient-centred care within the area of specialty.
• Lead and collaborate with Nurse Managers and Director of Nursing.
• Monitor clinical performance and develop and deliver education and policy updates.
• Implement changes to clinical practice and drive quality improvement.
• Support safe, person-centred care and engage with multidisciplinary teams.
