Queensland Health at Rockhampton Hospital is seeking a Clinical Nurse (Medical Care Coordinator) to lead advanced, patient‑centred nursing care for acute and complex medical conditions. This role coordinates discharge planning and works across medical, nursing, imaging, ambulance, and community services to ensure safe, timely transitions.

Registered Nurse with NMBA/AHPRA registration and acute medical nursing experience.
Advanced clinical judgement and problem-solving skills for patient care coordination.
Strong leadership, education and quality improvement experience.
Ability to lead discharge planning and care coordination across teams.

Responsibilities

Lead advanced nursing care for patients with acute and complex medical conditions.
Coordinate patient care and discharge planning for General Medical patients.
Provide clinical leadership across practice, education and research.
Champion quality improvement with governance and safety initiatives.
