Job summary

Step Up Supports is seeking a Client Clinical Case Manager (Registered Nurse preferred) for a full-time role in Ballarat. You will provide clinical oversight for client medical management and support staff through complex situations in the community and home environments.

You’ll participate in a rotating after-hours roster, including one evening per week and rotating weekend coverage, working with clients, families and the broader support team to achieve positive outcomes.

Qualifications

• Must hold an RN qualification and be eligible to practice.
• Experience in disability support and complex case management is required.
• Ability to use project software and coordinate across teams.

Responsibilities

• Clinically manage clients with complex needs with compassion and sound judgement.
• Oversee health plans, wound care, and daily medication management.
• Collaborate with clients, families, stakeholders and support teams.
• Read client shift notes to understand real stories.
• Handle requests from staff, clients and stakeholders with solutions.
• Participate in after-hours roster, including one evening per week and rotating weekend coverage.
• Work directly with clients in community and home settings.
• Support quality outcomes, risk management and positive team culture.
