Job summary

Client Centred Home Care on the Gold Coast is seeking an experienced Registered Nurse (RN) or Enrolled Nurse (EN) to join as Clinical Case Manager. You will lead client-centric care planning, coordinate high-quality multidisciplinary care, and oversee a team of community nurses and support workers.

You will collaborate with families, relatives, and support coordinators, ensure plans are followed and reviewed, and provide clinical guidance and escalation support to field staff.

Qualifications

• Registered Nurse (RN) or Enrolled Nurse (EN) with current registration.
• Experience in community or complex care settings.
• Excellent communication and stakeholder engagement skills.
• Highly organised with the ability to manage competing priorities.

Responsibilities

• Develop, implement, and review comprehensive care plans and high-intensity support plans as needed.
• Actively participate in stakeholder and multidisciplinary meetings.
• Liaise with family members, next of kin, and support coordinators.
• Oversee and support a team of community nurses and support workers.
• Ensure care plans are followed, reviewed regularly, and staff trained accordingly.
• Manage medication oversight, incident reporting, and clinical reviews.
• Provide clinical guidance and escalation support to field staff.
• Collaborate with head office and contribute to continuous improvement initiatives.

Southport - Monday to Friday, 8:00am – 4:00pm

Are you an experienced and passionate nurse ready to step into a dynamic, fast-paced role where no two days are the same? We are seeking a dedicated Clinical Case Manager (RN or EN) to join our team and play a pivotal role in delivering high-quality, client-centred care in the community.

About the Role
As a Clinical Case Manager, you will be the subject matter expert for your allocated clients, ensuring their clinical and holistic needs are met through effective planning, coordination, and leadership. You will be a key contributor in multidisciplinary and stakeholder meetings, advocating for your clients while working collaboratively with internal and external partners.

Key Responsibilities

Develop, implement, and review comprehensive care plans and high-intensity support plans (where required)

Actively participate in stakeholder and multidisciplinary meetings

Liaise with family members, next of kin, and support coordinators

Oversee and support a team of community nurses and support workers

Ensure care plans are followed, reviewed regularly, and staff are appropriately trained for each client

Manage medication oversight, incident reporting, and clinical reviews

Provide clinical guidance and escalation support to field staff

Collaborate closely with head office and contribute to continuous improvement initiatives

About You

Registered Nurse (RN) or Enrolled Nurse (EN) with current registration
