Right at Home in Australia is seeking a Registered Nurse to join the Support at Home team as a Clinical Care Partner. You will provide clinical leadership, coordinate care, and oversee a caseload of home-based clients, ensuring safe and person-centred services. autonomy and team collaboration are key.
Ideal candidates have strong assessment, planning and leadership skills and can travel to clients' homes across the Sunshine Coast region.
Qualifikationen
- Must hold current AHPRA RN registration.
- Experience in community nursing, aged care, rehab or palliative care.
- Strong clinical assessment and care planning capability.
- Understanding of Support at Home classifications, funding and service delivery.
- Experience supervising or guiding care staff.
- Driver's licence and reliable vehicle, and police clearance.
- Ability to travel to clients' homes independently.
Aufgaben
- Provide clinical leadership and oversight for clients in the Support at Home program.
- Manage caseload, conduct assessments and develop care plans.
- Support care staff and coordinate services to meet budgets.
- Be a trusted clinical contact for clients, families and MDT partners.
- Ensure care is safe, coordinated and person-centred.
