Job summary

Enabling Pathways Community in Cairns seeks a Clinical Care Coordinator - Registered Nurse to provide clinical oversight across disability support services, ensuring health needs are understood, documented and supported.

The role focuses on guidance, training and quality assurance, visiting SIL properties, coordinating care plans, and collaborating with GPs and staff to ensure safe, person-centred care. You will bring RN registration with AHPRA and strong communication skills.

Qualifications

• Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
• Experience in clinical assessment, care planning, medication safety and clinical documentation.
• Ability to identify clinical risks and escalate when needed.
• Strong communication skills to explain procedures to staff.

Responsibilities

• Provide clinical guidance across SIL, Community Access and disability services.
• Review health needs and care plans to support safe care.
• Develop and review clinical protocols and health management plans.
• Support staff with complex health needs and high-intensity activities.
• Visit SIL properties to review practices and provide guidance.
• Liaise with GPs and allied health professionals for coordinated care.
• Provide direct nursing care within scope of practice when required.
