Job summary

Right at Home Central Queensland is seeking an empathetic RN / Care Coordinator to ensure clients receive the right care at the right time in their own homes. You will coordinate in-home services, perform assessments, and build strong relationships with clients and families across the Rockhampton region.

The role requires AHPRA registration, a current driver’s license, and experience in aged care or disability support.

Qualifications

• AHPRA Registered and practising.
• Drivers Licence.
• Any other Aged Care / Health Care related certificates.
• Proven experience in aged care, disability, or mental health within a community setting.
• Strong understanding of Aged Care and Disability Standards, as well as consumer‑directed care principles.
• Current First Aid, CPR certification, and National Police Check.
• Working With Children (WWC) check or willingness to obtain.
• A valid driver's license and access to a reliable car with comprehensive insurance.

Responsibilities

• Coordinating and overseeing in-home care services.
• Conducting care assessments, reviews, and service planning.
• Building strong relationships with clients, families, carers, and the wider care team.
• Monitoring care delivery to ensure quality, safety, and compliance.
• Supporting carers and staff with guidance, training, and regular updates.
• Travelling throughout the Wide Bay and surrounding areas— with a company car provided for work‑related travel.

About the Role
