Job summary

Right at Home Central Queensland is seeking a compassionate RN / Care Coordinator to join our local team in Hervey Bay and wider Wide Bay region. You will coordinate in-home care services, conduct care assessments, and plan services that deliver dignity, safety, and comfort for clients in their own homes.

You'll build strong relationships with clients, families and carers, support our care team with guidance and training, and travel as required with a company car provided.

Qualifications

• AHPRA registered and practicing.
• Experience in aged care, community care or disability support.
• Current driver's licence and access to a reliable vehicle.
• Knowledge of Aged Care and Disability Standards and consumer-directed care.
• First Aid/CPR certification; WWC check or willingness.

Responsibilities

We’re looking for a caring, reliable, energetic, and organised RN / Care Coordinator to become part of our growing team and help us deliver exceptional care across our local community.

About the Role

As our RN / Care Coordinator, you’ll play a key role in making sure our clients receive the right care, at the right time, in the comfort of their own homes.

Your day-to-day will include:

• Coordinating and overseeing in-home care services
• Conducting care assessments, reviews, and service planning
• Building strong relationships with clients, families, carers, and the wider care team
• Monitoring care delivery to ensure quality, safety, and compliance
• Supporting carers and staff with guidance, training, and regular updates
• Travelling throughout the Wide Bay and surrounding areas— with a company car provided for work-related travel

You’ll ideally bring:

• Previous experience in Aged Care, Community Care, or Disability Support
• Strong time management and organisational skills
• Excellent communication and interpersonal skills
• Knowledge of care standards, assessments, and service coordination
• A current driver's licence and access to a reliable vehicle

• AHPRA Registered and practicing
• Drivers Licence
• Any other Aged Care / Health Care related certificates
• Proven experience in aged care, disability, or mental health within a community setting.
• Strong understanding of Aged Care and Disability Standards, as well as consumer-directed care principles.
• Current First Aid, CPR certification, and National Police Check.
• Working With Children (WWC) check or willingness to obtain.
• A valid driver's license and access to a reliable car with comprehensive insurance.
• Knowledge of Home Care Packages (HCP) or Support at Home (SAH) (Desirable)
