Complete Care Team in Belrose is seeking a Registered Nurse to join as an RN Service Coordinator. You will liaise with case managers, medical teams, families and clients to ensure high-quality, at-home care.

This full-time role involves travelling across Sydney and NSW, providing clinical support, developing care plans, training care workers, and ensuring compliance with WHS and company policy; career progression is available.

Qualificações

• Registered nurse with current registration and Australian authority.
• Experience in home-based care or disability support preferred.
• Leadership and mentoring abilities for care workers and nurses.
• Strong communication with clients, families and case managers.
• Willingness to travel across Sydney and NSW.

Responsabilidades

• Regular evaluation of client’s services.
• Develop and update client Individual Support Plans.
• Liaise with Case Managers and other professionals.
• Support day-to-day management of client needs.
• Ensure WHS compliance and company policies.
• Provide training and clinical support to care workers.
