Job summary

Horizon Therapy Services Pty is seeking a committed NDIS Behaviour Support Practitioner to assess, plan, and supervise behaviour support for clients across Australia. You will work flexibly in a hybrid model with a focus on person-centred, trauma-informed practice and collaboration with families and support teams.

The role emphasizes data-driven planning, regular coaching of caregivers, and maintaining rigorous clinical documentation within NDIS standards.

Qualifications

• Suitable as NDIS Behaviour Support Practitioner Core level or above.
• Qualifications in behaviour support, disability, psychology or related discipline.
• Knowledge of Positive Behaviour Support and functional assessments.
• Ability to develop Individualised Behaviour Support Plans per NDIS.
• Strong clinical reasoning to inform interventions.
• Excellent communication; remote consultations and collaboration.
• Independent worker with caseload management and documentation.

Responsibilities

• Conduct comprehensive functional behaviour assessments.
• Identify contributing environmental and social factors.
• Develop Interim and Comprehensive Behaviour Support Plans under NDIS.
• Set meaningful, measurable goals with clients and networks.
• Schedule regular consultations with clients and caregivers.
• Monitor behaviour frequency and progress.
• Interpret data and adjust interventions.
• Coach caregivers to implement strategies.
• Create practical resources for caregivers and staff.
• Maintain compliant clinical documentation.
