Talk Health Therapy is hiring a Triage Officer to be the first contact for CHSP aged care clients. This remote, casual position offers flexible hours and 2–4 days per week, with duties including initial assessments and data entry in Splose.

You will coordinate with clinical staff, maintain accurate records, and ensure privacy and confidentiality in all client interactions while using Outlook and Teams for team collaboration.

Formación

• A healthcare qualification with client-facing experience is required.
• Experience in customer service and client satisfaction is preferred.
• Ability to enter data accurately and manage records.
• Familiarity with healthcare systems like Splose or quick learner.

Responsabilidades

• Serve as the first point of contact for CHSP clients, managing inbound referrals and inquiries with empathy and professionalism
• Conduct initial clinical assessments, informing therapy planning and care coordination by our clinical team
• Assess and document client medical history, falls history, risk factors, living arrangements, and intake data
• Perform data entry and client record management using Splose
• Update and maintain internal database to manage client flow and follow-up requirements
• Coordinate with allied health professionals, clinical staff, and management to ensure timely care
• Communicate through Outlook and Teams for seamless coordination
• Uphold privacy, confidentiality and documentation accuracy in all interactions

Enrolled Nurse (EN) or Registered Nurse (RN) with AHPRA

Certificate III or IV in Allied Health Assistance

Allied health qualification (Physiotherapy, Occupational Therapy, Speech Pathology, Dietetics, Podiatry, or Exercise Physiology)
