Job summary

ForHealth Group in Australia is seeking a dedicated Registered Nurse or Enrolled Nurse to join as a Chronic Disease Management Health & Wellbeing Nurse. You will work closely with GPs and allied health teams to support patients, provide coaching, and coordinate care across services.

Qualifications

• Current AHPRA registration as a Registered Nurse (RN) or Enrolled Nurse (EN).
• Experience in Chronic Disease Management, Care Plans and Health Assessments preferred.
• Experience in General Practice or Primary Healthcare preferred.
• Strong understanding of preventative health and chronic disease principles.
• Excellent communication and interpersonal skills.
• Strong organisational and time management abilities.

Responsibilities

• Support patients to manage chronic conditions and achieve positive health outcomes.
• Provide health coaching and promote positive behaviour change initiatives.
• Coordinate care with GPs, specialists, allied health providers and community services.
• Conduct and coordinate clinical services including ECGs, Spirometry, ABI testing and DMMRs where appropriate.
• Maintain accurate patient records and ensure Medicare compliance requirements are met.
• Proactively identify eligible patients for chronic disease management and preventative health services.
• Build strong relationships with patients and provide ongoing education and support.

About The Role

About You

• Strong organisational and time management abilities
• The ability to motivate and engage patients in improving their health outcomes
• A proactive, patient-focused approach to care
