Job summary

ACH Group is seeking passionate Registered Nurses to join our Community Nursing Teams based at Christies Downs and Elizabeth South. This community-based role offers autonomy, variety and the chance to support older people to live independently at home.

You will conduct assessments, develop care plans, provide wound care and medication management, observe wellbeing and liaise with families and allied health professionals to support customers.

Qualifications

• Current nursing qualification with valid registration as a Registered Nurse (AHPRA).
• Experience in community or home-based care is preferred.
• Ability to work autonomously and manage multiple clients.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct assessments, reviews, coordinate care plans for nursing interventions tailored to individual customer needs.
• Provide nursing interventions such as wound care, medication management, continence care, and management of chronic conditions
• Observe and evaluate customer wellbeing, making timely and appropriate actions in response to clinical changes.
• Support the wellbeing of customers living in their own home
• Document nursing care accurately, thoroughly, and in a timely manner
• Participate in on-call after hours service

What you'll do:

• Support the wellbeing of customers living in their own home

• Document nursing care accurately, thoroughly, and in a timely manner
• Participate in on-call after hours service
• Liaise with internal and external stakeholders including family members, Support Workers and Allied Health Professionals to support customers

About You!

You are a talented and passionate Registered Nurse who enjoys variety, values connection and wants to use your clinical skills in a meaningful way. You may already have community experience, or you may be ready to bring your nursing expertise into a new environment where every day looks a little different.

• Demonstrate significant experience as an Registered Nurse nb, preferably in community setting
• Excellent attention to detail
• Excellent interpersonal, communication and problem-solving skills
• Ability to work autonomously
• Intermediate computer skills
• Availability to work on call
• Available to work at least 3 days per week

You will need:

• A nursing qualification and current Registered Nurse registration with AHPRA
• A current driver's license and reliable vehicle
• A satisfactory police clearance for working with vulnerable people that is less than 12 months old before commencement
