Job summary

AnglicareSA is seeking an experienced Registered Nurse to deliver high-quality, person-centred community nursing in South Australia. You’ll assess, plan and coordinate care for clients in their own homes, including higher-level Support at Home, reablement and end-of-life services.

You will work collaboratively with clients, families and health professionals to respond to changing clinical needs, provide expert guidance and ensure timely delivery of services.

Qualifications

  • Bachelor of Nursing and current AHPRA registration as a Registered Nurse required.
  • Minimum 3 years' post-registration experience (acute hospital and/or community setting).
  • Strong experience in wound management and exposure to palliative or end-of-life care.

Responsibilities

  • Conduct comprehensive clinical assessments and reviews to inform high-quality care planning.
  • Manage escalated and complex cases including restorative, short-term and palliative care.
  • Provide wound management expertise and clinical advice to multidisciplinary teams.
  • Prioritise and manage a dynamic caseload in a fast-paced community environment.
  • Support high clinical governance standards through accurate documentation and best-practice nursing care.

Join us as a Registered Nurse and bring your clinical skills into the community where relationships and continuity of care truly matter.

What you'll do

As a Registered Nurse within our Community Aged Care team, you'll provide high-quality, person-centred nursing care that supports older South Australians to live safely, independently and well in their own homes.

You'll use your clinical expertise to assess, plan and coordinate care for customers with a range of health needs, including those requiring higher-level Support at Home, reablement, restorative and end-of-life care.

Working collaboratively with customers, families, care teams and other health professionals, you'll respond to changing and escalated clinical needs, provide expert nursing guidance and help ensure the right care and services are delivered at the right time.

You’ll be trusted to make a valuable impact by:

  • Conducting comprehensive clinical assessments and reviews to inform high-quality care planning
  • Managing escalated and complex cases, including restorative, short-term and palliative care

About the Candidate

You're an experienced Registered Nurse with strong clinical judgement, a person-centred approach and the confidence to work independently in the community.

To thrive in this role, you will have:

  • Bachelor of Nursing and current AHPRA registration as a Registered Nurse
  • Minimum 3 years' post-registration experience (acute hospital and/or community setting)
  • Strong experience in wound management and exposure to palliative or end-of-life care
  • Demonstrated ability to manage competing priorities and work autonomously
  • Current driver's licence and access to a fully insured vehicle

We would also love you to have:

  • Community Aged Care or Support at Home experience
  • Experience in restorative, reablement or end-of-life care
  • Experience supporting or mentoring other staff
