Job summary

Juniper Annesley in Bentley, WA is seeking a Registered Nurse to join our supported residential aged care team. You will assess residents' clinical needs, coordinate care with healthcare professionals, and implement person-centred care plans.

We are looking for a compassionate RN with AHPRA registration and a minimum of two years' experience in aged care, who can guide staff in wound and diabetes management and maintain high standards of clinical care.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• 2+ years’ experience as an RN in an aged care setting (or equivalent).
• Strong clinical assessment and care coordination skills.
• Ability to guide staff in wound and diabetes management.
• Sound understanding of medication systems.
• Excellent communication, teamwork and problem-solving skills.

Responsibilities

• Conduct clinical assessments and respond to changes in resident health and wellbeing.
• Develop, implement and review person-centred care plans in partnership with residents and their families.
• Administer medications and provide clinical treatments in line with professional standards.
• Provide leadership, mentoring and guidance to care staff.
• Liaise with GPs, allied health professionals and external providers to coordinate resident care.
• Maintain accurate and timely clinical documentation and reporting.
• Support AN-ACC assessments and ensure accurate clinical documentation.
• Ensure compliance with Aged Care Quality Standards, policies and best practice.
• Participate in infection prevention, risk management and quality improvement initiatives.
