Surf Coast Superclinic in Winchelsea, Victoria, is seeking a compassionate Registered Nurse to join our general practice team. The successful candidate will provide clinical nursing services, preventative health initiatives, and chronic disease management while supporting our General Practitioners and allied health professionals.

Responsibilities include health assessments, vaccinations, care planning, recalls, and patient education in a modern, patient-centred environment.

Qualifications

• Current nursing registration with AHPRA as a Registered Nurse.
• Current CPR Certificate.
• Proficient computer and patient management software skills.

Responsabilités

• Provide high-quality nursing care within a general practice setting.
• Conduct health assessments, care planning, and patient screenings.
• Assist with chronic disease management and care coordination.
• Administer vaccinations and maintain cold chain management.
• Perform wound care, dressings, and infection control procedures.
• Undertake ECGs, spirometry, and other clinical assessments.
• Support GP management plans and health assessments.
• Maintain accurate patient records using clinical software.
• Manage recalls, reminders, and preventative health programs.
• Participate in quality improvement initiatives and accreditation requirements.
• Educate patients on health promotion and disease prevention.

About the Role

We are seeking a compassionate and experienced Registered Nurse to join our growing general practice team. The successful candidate will play a vital role in providing clinical nursing services, preventative health initiatives, chronic disease management, and patient education while supporting our General Practitioners and allied health professionals.

Key Responsibilities

• Provide high-quality nursing care within a general practice setting.
• Conduct health assessments, care planning, and patient screenings.
• Assist with chronic disease management and care coordination.
• Administer vaccinations and maintain cold chain management.
• Perform wound care, dressings, and infection control procedures.

Essential Requirements

• Current registration with AHPRA as a Registered Nurse.
• Current National Police Check.
• Current CPR Certificate.
• Excellent communication and interpersonal skills.
• Strong clinical assessment and organizational skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Proficient computer and patient management software skills.

Desirable Criteria

• Previous experience in General Practice nursing.
• Immunisation Certificate and experience with vaccination programs.
• Chronic Disease Management experience.
• Women's Health, Health Assessments, and Care Planning experience.
• Knowledge of Medicare item numbers and Practice Incentive Programs (PIP).
• Experience with Best Practice or similar clinical software.
