Job summary

Mercy Health is seeking a casual Community Nurse to provide high-quality, patient-centered care to older adults in their homes in the Newcastle area. You will balance hands-on nursing tasks with collaboration from the care management team to ensure safe, effective home-based care.

The role requires registration with AHPRA, a minimum of 2 years’ community nursing experience, and a current Police Check and Working with Children Check.

Qualifications

• Bachelor of Nursing with current AHPRA registration.
• Minimum of 2 years’ experience working as a Registered Nurse in the community.
• Current Police Record Check and Working with Children Check.

Responsibilities

• Provide wound care, catheter care, medication administration and health assessments in the community.
• Deliver high-quality, individualized patient-centered care in older adults’ homes.
• Collaborate with Clinical Lead and Care Managers to coordinate client referrals.

About the opportunity

Apply your nursing skills in a community setting where your care and dedication will be valued and respected. You will also have the time and support to deliver individualised, high-quality care to older people living at home.

This rewarding position offers:

• A nursing role helping older people live independently at home.
• A great balance of hands-on nursing care and working directly with the support of our exceptional care management team.
• Opportunity for flexible work arrangement
• Attend to community nursing services including the provision of wound care, catheter care, medication administration and health care assessment using validated assessment tools.
• Undertake health care assessments and make client referrals and recommendations in collaboration with the Clinical Lead and client Care Managers.

You will also be required to provide evidence of, or in the process of obtaining:

• Bachelor of Nursing with current AHPRA registration and minimum of 2 years’ experience working as a Registered Nurse in the community.
• A current Police Record Check and Working with Children Check.
