Settlement Services International (SSI Group) invites applications for a SRSS Case Manager position based in Dandenong, Victoria. You will deliver holistic, client-centred case management to newly arrived refugees and asylum seekers under DHA contract.

As part of a team of up to 7 case managers, you will conduct needs and risk assessments, support exit planning, and maintain accurate records in the SRSS Client Database, while upholding SSI policies and collaborating with the team.

Qualificações

• Diploma/Degree in Community Services/Welfare or related discipline.
• Minimum two years’ casework experience.
• Ability to work independently on general casework tasks with minimal supervision.
• Understanding of case management approaches and theoretical frameworks.
• Experience working with CALD clients.

Responsabilidades

• Develop holistic and client centred case plans with involvement of the client/s.
• Ensure appropriate exit planning and transition to post SRSS support within an appropriate timeframe.
• Undertake comprehensive and regular needs and risk assessments of clients against the vulnerability criteria for the SRSS program
• Support the clients to develop independent skills and to access appropriate supports in their community
• Organise case conferences for clients with complex needs
• Maintain accurate digital records of client interactions and services within the SRSS Client Database in line with SSI policies and procedures
• Ensure consistently high-level service is provided to asylum seekers through teamwork and support to SRSS colleagues
• Prepare client for exit from the service and establish closure with the client
