Job summary

Settlement Services International (SSI Group) in Victoria is seeking a Case Manager to provide holistic support to newly arrived refugees within the SRSS program under DHA contract. You will deliver face-to-face and remote services to vulnerable clients, including families and adults.

Qualifications

• Diploma or degree in Community Services/Welfare or related discipline.
• Minimum two years’ casework experience.
• Ability to work independently with minimal supervision.
• Understanding of case management approaches and frameworks.
• Experience working with CALD clients.

Responsibilities

• Develop holistic, client-centred case plans with client involvement.
• Ensure exit planning and transition to post-SRSS support.
• Undertake comprehensive needs and risk assessments per SRSS criteria.
• Support clients to access community supports and build independent skills.
• Organise case conferences for clients with complex needs.
• Maintain accurate digital records per SSI policies.
• Ensure high-level service through teamwork and support to SRSS colleagues.
• Prepare clients for exit and closure of the service.
