Job summary

Victorian Government seeks an assessment and transition coordinator for the Department of Justice and Community Safety. You will assess prisoners, connect them with community programs and coordinate reintegration efforts to support transitions after release.

You will work with prison staff and service providers to arrange referrals, maintain records, and contribute to parole planning decisions, ensuring safety and successful community re-entry.

Qualifications

• Tertiary qualification in a relevant field is highly desirable.
• Experience in human services, welfare, education or related field is also highly desirable.
• Knowledge of prisoner transition and reintegration is beneficial.

Responsibilities

• Assess prisoners' risk and needs.
• Help prisoners access programs and services supporting transition to the community.
• Coordinate reintegration programs and referrals with prison staff and service providers.
• Maintain accurate records and program data to inform service planning and review.
• Support parole planning and release decisions through information sharing with Community Correctional Services.
